nursE CONSULTing Services

Having the support and advocacy of a registered nurse to help you understand the options of care available in your community will help you make more informed decisions about the care that you or your loved one needs. This is particularly true as you age, or experience the impacts of chronic or terminal illness . Nurse case management is also available as part of this level of service. 

Nurse Consultation Services are for you if you:

• would like professional support in anticipating and planning for the changing care needs of yourself or a family member, including help facilitating family discussions
• want to understand and locate the different care options in your community
• need assistance in facilitating changes in level of care, such as transitioning to assisted living
• want nurse case management that includes but is not limited to, scheduling and coordinating appointments (medical and non-medical), creating care schedules for caregivers, and providing patient advocacy

Remote Service:

Consultation services are available online via video conference.  If you live in the Northern Humboldt area, nurse consultation may be available in person, according to current Covid-19 CDC recommendations.  Please contact for more information.
