An end-of-life plan is a concise and clear document (in a workbook format) that helps bring peace of mind to you and your family/loved ones. This plan enables you to clearly articulate how you want the practical aspects of your life to be handled at the end of your life, and also when you are no longer here. Sometimes people avoid having these frank discussions with family members, and having professional assistance in creating this plan can help ensure its completion.

During our initial, 2 hour meeting, you will receive the actual end-of-life planner and we will discuss your main goals and concerns, as we review the document and begin gathering information for its completion. Follow-up sessions are available to help support you in completing the plan, as needed.
Remote Service:
This service is available online via video conference. If you live in the Northern Humboldt area, this service may be available in person, according to current Covid-19 CDC recommendations. Please contact for more information.
